dc.description.abstractWe aimed to evaluate the prognostic performance of bilateral ASPECTS (ASPECTS-b) to predict neurologic outcome in post cardiac arrest patients treated with targeted temperature management (TTM). We conducted a retrospective study of post cardiac arrest patients treated with TTM in two tertiary hospitals. The ASPECTS-b and GWR on brain computed tomography (CT) achieved within 6 hours of return of spontaneous resuscitation (ROSC) was measured twice from each institute. ROC curve analysis of ASPECTS-b and comparison of the ROC curve from each institute was done to assess the performance in predicting poor neurologic outcome (CPC 3-5). A total of 66 patients from institute A and 109 patients from institute B was studied. Of the total of 175 patients enrolled, 117 patients (66.9%) had poor neurologic outcome. ASPECTS-b showed good prognostic performance to predict poor neurologic outcome [AUROC 0.72 (0.64-0.80)] and was not significantly different between institutes [institute A vs. institute B, AUROC 0.83 (95%CI 0.72-0.94) vs. AUROC 0.73(95%CI 0.64-0.83), p=0.2]. ASPECTS-b measured from initial brain CT not only expressed good performance in predicting poor neurologic outcomes in post cardiac arrest patients, but showed similar performances between two different institutes.-
dc.description.abstract본 연구는 심정지 후 저체온요법을 시행 받은 환자의 조기 신경학적 예후를 예측하는데 있어, 양측성 ASPECTS (ASPECTS-b)의 예후 예측 능력을 보고자한다. 두개의 대학병원에 내원한 병원밖 심정지 후 환자들을 대상으로 후향적인 연구를 시행하였다. 두기관에서 자발순환회복 후 6시간이내 촬영한 뇌전산화단층촬영에서 ASPECTS-b와 회백질비율 (GWR)을 각각 2회씩 측정하였다. ASPECTS-b의 나쁜 신경학적 예후(CPC3-5) 예측에 대한 ROC curve 분석을 시행하였고, 각 기관의 ROC curve를 비교하였다. A 기관에서 66명의 환자와 B기관에서 109명의 환자가 연구에 포함되었다. 총 175명 중, 117명(66%)가 나쁜 신경학적 예후를 가지게 되었다. ASPECTS-b는 나쁜 신경학적 예후를 예측하는데 좋은 성적을 보였으며[AUROC 0.72 (0.64-0.80)] 이는 두기관 간 유의하게 다르지 않았다[A기관 vs. B기관, AUROC 0.83 (95%CI 0.72-0.94) vs. AUROC 0.73(95%CI 0.64-0.83), p=0.2]. 조기 뇌전산화단층촬영에서 측정한 ASPECTS-b는 심정지 후 환자들의 나쁜 신경학적 예후를 예측하는데 좋은 성적을 보였으며, 두 기관간 비슷한 성적을 보였다.-
